In addition to our outpatient clinics we also own and operate two residential treatment facilities for individual with severe and persistent mental illness who also have medical conditions.
Assertive Community Treatment, or ACT is an integrated approach for community mental health service delivery. ACT serves outpatients whose symptoms on mental illness result in functioning difficulties in life, work, social relationships and physical health and wellness. Supported Employment is another integrated approach to folks offering competitive employment for those suffering a disabling condition. Employees are matched specifically to their job description and to their job culture. Services offered: Job Assessment, Job Match, Benefits Counseling, Job Development, and Long Term Support.
We have successfully implemented a model for integrating wellness that incorporates enhanced care coordination of services between mental health/drug and addiction services and primary health care. As part of this collaborative approach to healthcare, Winding Waters Medical Clinic now shares space at our Annex location offering full medical services within our behavioral health home. We also have staff who have been trained as Behavioral Health Coaches and are working as an integrated team with primary care. Our consumers are already realizing the benefits. This approach provides greater access to care and better health outcomes for people.
